You must complete a qualifying activity within the next few days or you will not receive your PHMP benefit this month.

How do I complete a qualifying service?

Visit the PHMP Member Portal and get started! It’s fast, easy and there are several options in which you can engage in the program from the comfort of your own home, including:

  • Start a fitness log
  • Take online lessons in exercise, health education, nutrition, personal development or stress management
  • Contact a PHMP Health Coach and speak to a health care professional about your individualized plan
  • Feeling under the weather? Talk to a medical professional anytime, anywhere, over the phone using the RealTimeTelemed service
