Let's Get You Started
Registering Is Easy
After you sign on as a new user, we encourage you to utilize our simple, user-friendly website for all of your Proactive Health Management Plan needs. And there two ways to register: via computer or via the app.
Follow the instructions below to get started.
Register On The App
Download & Open the App
Download the app on your device by clicking one of the options below:
Create An Account
On the CREATE A NEW ACCOUNT screen enter the following information found on your ID Card:
- Group Number
- Social Security Number
- Date of Birth
- Last Name
Then tap the SIGNUP button.
Register
Next, create a username and password, enter your email and read and ACCEPT the legal notice.
Click REGISTER.
Register On Your Computer
Access The Website
Go to phmp.myezhealthguide.comComplete Registration
Enter your social security number without dashes, and use the format MM/DD/YYYY for your birthdate.
Then click the SIGN UP button.
Review & Accept Terms of Service
After reviewing, check off the I AGREE box, then click the REGISTER button.
